Sharp HealthCare is participating in a new program from Medicare called an Accountable Care Organization (ACO). An ACO is a group of doctors, hospitals and other health care providers working together to give you better service and care.
The goal is to provide you with improved health and a better patient experience, particularly if you're ever hospitalized or if you're living with a chronic condition. We will reach out to you to ensure you receive important exams and immunizations, and that you're receiving the preventive care you need when you need it.
This enhanced care coordination program began on Jan. 1, 2012. Sharp HealthCare patients who are part of this program have been notified via a letter in the mail.
