Registration and Security
You will be required to register for an account to apply for a job at Sharp HealthCare online or access mySharp. A user name and password will be needed to register. You will be asked to provide information pertinent to your account. You are responsible for securing the confidentiality of your password. When choosing a password, select a combination of letters and numbers that isn’t guessable by someone who knows you. Examples of weak passwords include those that incorporate your family member’s and pet’s names, sports teams or hobbies or license plate number. Sharp will secure mySharp and job application submissions using SSL (Secure Socket Layer).
You must be at least 18 years old to register for a mySharp account or apply for jobs online. We do not knowingly allow users under 18 to have their own accounts. You may cancel your account at any time by notifying Sharp HealthCare at email@example.com. Sharp HealthCare reserves the right to cancel or remove accounts that impersonate another person, are abusive, illegal or are otherwise inappropriate, or which are used to violate any part of this agreement.

mySharp Sign-Up Process
While signing up for mySharp, you will be asked to provide your Social Security number and other personal information to connect your mySharp account with your or your loved one's medical records.
For Sharp Rees-Stealy patients who would prefer not to provide their full Social Security number on the sign-up form, we offer an alternative sign-up option. You may visit a Sharp Rees-Stealy location or call us at 858-499-2795. A customer service representative will locate your patient record and email you a link to sign up. For your security, you will be prompted to enter the last four digits of your Social Security number so we can verify your identity.
Non-Sharp Rees-Stealy patients who prefer not to provide a Social Security number will need to print and fill out a form, have it notarized and mail it to: Sharp Rees-Stealy Health Information Management, 2929 Health Center Drive, San Diego, CA, 92123. At this time, this is the only way that we can verify your identity if you are not already in our system. If you have any questions, call us at 619-585-4088.
Identity Verification Process
mySharp's identity proofing process is designed to protect patient privacy and prevent identity theft. It requires you to answer a set of specific questions before gaining access to your records for the first time. We are working with a national credit agency to verify your identity. This agency selects questions based on information found in public and semi-public records. Sharp HealthCare will never see your answers. If the agency is unable to verify your identity through this process, we will ask you to call our Technical Assistance Center at 858-627-5201.
After going through the identity verification process, you may notice an inquiry on your credit report. This type of inquiry is known as a “soft inquiry.” It occurs when a business checks your credit for reasons not involving a credit application. This “soft inquiry” is not viewable to lenders and will not affect your credit score.

Logging in to mySharp
Username and Password
When creating a mySharp account, you will be asked to select a username and password. You may change your password at any time by visiting “mySharp Account Info.” If you forget your username or password, please contact firstname.lastname@example.org or call 858-627-5201.
When you log in to mySharp, a security pass code may be sent to your phone. You will need to enter this pass code in order to access your account. Pass codes expire 15 minutes after they are sent.

Managing Other Accounts Through mySharp
Once signed up for mySharp, you may request access to the health information of your minor child or an adult for whom you act as a conservator. To complete this request, visit the Manage mySharp Settings section. You may also get access to another person’s Sharp Rees-Stealy account if the patient or the patient’s parent/legal guardian/conservator grants you access.
If you get access to a patient’s protected health information through mySharp, you will be responsible for maintaining the privacy of the information in the account. You release Sharp from any/all liability that may arise from granting you access to this information. This covers all patient information that will be available through mySharp, including information that may relate to the patient’s HIV status, psychiatric care and/or treatment for drug and alcohol abuse.
You may give up your access at any time by using the “Remove” column on the Manage My Accounts page. The revocation is effective upon its receipt by Sharp. Please understand that revocation cannot apply to information disclosures that have already taken place prior to the revocation.
By using mySharp, you take responsibility for periodically checking the Manage My Accounts page in mySharp to see which accounts you can access.

Granting Access to a mySharp Account
If you are a mySharp user with Sharp Rees-Stealy Medical Group, you may grant up to four adults access to your mySharp account – two with full access and two with read-only access. You may also grant access to the Sharp Rees-Stealy account of a child or adult for whom you act as a guardian or conservator. At this time, you may only grant access to people who are also Sharp Rees-Stealy patients.
If you grant someone access to your account or your child/dependent’s account, this person will have access to all health information available in the account. You release Sharp from any/all liability that may arise from granting the person access to protected health information. This covers all patient information that will be available through mySharp, including information that may relate to HIV status, psychiatric care and/or treatment for drug and alcohol abuse. By granting another person access to your account or the account of a child or dependent adult, you are giving Sharp physicians and staff authority to communicate with this person regarding the patient.
The decision to permit access to someone else is purely voluntary and will have no other affect on the care that is available to you. Please be aware that once the information leaves Sharp, Sharp will no longer be able to protect that information, and that recipients of the information (including the person you designate) may not be legally required to protect it.
You may remove a person’s access to your account at any time by using the “Remove” column on the Manage My Accounts page. To remove someone’s access to your child/dependent’s account, click on the “Update who can see…” link on the Manage My Accounts page and use the “Remove” column. The revocation is effective upon its receipt by Sharp. Please understand that revocation cannot apply to information disclosures that have already taken place prior to the revocation.
By using mySharp, you take responsibility for periodically checking the Manage My Accounts page in mySharp to see who has access to your account and the accounts of your children/dependents.

Secure Messaging Through mySharp
The Messaging tool allows you to send secure email to Sharp Rees-Stealy doctors' offices. Messages are reviewed during business hours and are typically answered within two business days. Your doctors' offices also have the capability to initiate messages. A notification will be sent to your personal email when there is new mail waiting in your mySharp inbox.
Please understand that messages sent through mySharp may be viewed by anyone from the doctor's office (office staff, nurses, other doctors, etc.). This allows us to provide a quicker response time for messages. Your message and the response from the doctor's office may become part of the permanent medical record.
This tool is for nonurgent health issues only. If you have a life- or limb-threatening emergency, dial 911 immediately. If you have an issue that needs to be addressed right away, please call 858-499-2795 to speak to a Sharp Rees-Stealy representative.

Viewing Lab Results Through mySharp
If you are expecting Sharp Rees-Stealy lab test results, please check mySharp’s “View Lab Results” section. After testing, allow one week for results to be posted.
You will be able to view results from lab tests that were performed at any Sharp Rees-Stealy location up to 30 days prior to your mySharp sign-up date.
If you are managing someone else's Sharp Rees-Stealy health information, you will see results from lab tests performed on or after the day you gained access to that patient's mySharp account.
Lab Tests That Won't Display on mySharp
There are some limitations to what test results may be displayed online. Tests that will not display on mySharp include those that are sent out to non-Sharp labs for processing. Additionally, for legal and privacy reasons we will not display certain results related to:
- Drug abuse
- Pathology tests (including Pap tests, biopsies, tests involving products of conception and certain bone marrow tests)
- STDs (sexually transmitted diseases)
Due to California privacy laws, we are currently unable to provide lab results for children between the ages of 12 and 17.

Collection and Use of Personal Information
Sharp HealthCare will not collect your name, email address or other personal information unless you voluntarily provide it. Sharp will not sell, trade, rent or share personal information about you or other website visitors to any third party. Sharp follows industry best practices for data security.
Sharp collects, uses and stores the following personal information:
1. Names and email addresses for email newsletter subscriptions, online surveys, customer feedback and contact forms, and other communications with Sharp HealthCare. Sharp HealthCare will not send unsolicited email messages to you or other website visitors. Email messages sent to Sharp.com users are strictly limited to email newsletter subscriptions, mySharp account information and alerts, responses to questions or other uses in which the user has specifically requested or initiated the message.
2. Personal information (such as but not limited to name, email address, billing and shipping address, date of birth and date of health care service) when you register for a class, pay a bill, make an online donation, request a prescription refill, purchase a product, take a survey or participate in other online functions.
3. Credit card information to pay a bill, make an online donation or purchase a product. Sharp does not store credit card numbers. All credit card transactions are secured and processed using the standard practices of banks and retail stores.
4. Demographic information (such as but not limited to gender and ethnicity) is optional but requested when you apply for a job.
5. Information from your medical record will be displayed in mySharp if you elect to sign up for this service.

Collection and Use of Anonymous Information
By using Sharp.com, you consent to the collection and use of information as discussed below. Changes in this policy will be posted on this page so you will always know what information is being collected, how it is being used and under what circumstances it is being disclosed.
Sharp HealthCare collects the following nonpersonal information about its website visitors on an aggregated basis:
