If you or a loved one have fallen ill, suffered a disability or had your daily life interrupted in a way that makes it hard to continue to live alone, the Project CARE program at Sharp Coronado Hospital is here to help.
Project CARE (Community Action to Reach the Elderly) is sponsored by the San Diego County Health and Human Services Department, the city of Coronado and Sharp Coronado Hospital and is offered to Coronado participants at no cost. The program assists elderly and disabled adults who live alone.
Sharp Coronado’s program, created in 2000 and funded by the three cooperative sponsors, provides unique services to participants including:
Are You OK?
Calls are placed seven days a week at a time chosen by the participant. During the week, responses are monitored by the hospital’s patient relations staff, while on weekends responses are monitored by hospital security personnel. If a call goes unanswered, Coronado Police patrol volunteers are quick to act and will check on participants at their residences.
Vial of Life
Each participant is provided with a small container that holds health-related information such as the participant's name, address, and medication history. Should paramedics arrive in an emergency, this information assists them in providing a fast and accurate medical response.
The Sharp Coronado Community Pharmacy is located adjacent to Sharp Coronado Hospital and provides a convenient way to refill prescriptions and print a medication history. To contact a pharmacist or obtain more information, please call 619-522-3996 or e-mail communitypharmacy@sharp.com.
