How to Enroll in the Diabetes Education Program

Making it easy for you to access Sharp's diabetes services.

The Diabetes Self-Management Training Center at Sharp HealthCare, which is recognized by the American Diabetes Association, offers outpatient and support services to help people with diabetes to learn the skills needed to successfully self-manage diabetes and live a long, healthy life.

To access the services provided by Sharp’s Diabetes Self-Management Center, you will need a referral from your physician. Download a copy of the appropriate referral form below:

A diabetes educator will meet with you to assess the areas of need for diabetes education and collaborate with you to develop an individualized education plan. It will be determined whether one-on-one or group instruction is most appropriate for your needs.

Diabetes services are available at Sharp Chula Vista Medical Center, Sharp Grossmont Hospital, Sharp Memorial Outpatient Pavilion and Sharp facilities throughout the San Diego County via Sharp Rees-Stealy Medical Group.
