Presented by Sharp Chula Vista Medical Center

If you will be providing care for a loved one who has recently left or is getting ready to leave the hospital, you may have questions about what to expect when you both return home.

Our expert health care providers at Sharp Chula Vista Medical Center will cover key topics including:

  • Care options after being discharged from the hospital — community services and resources such as skilled nursing facilities and home health care options
  • Reducing the risk of falls — health and environmental causes, improving home safety
  • Providing proper nutrition — how to eat well at home, foods that are easy to prepare and store, community resources such as food pantries and home delivery programs
  • Understanding medications — the "Three A's" (Access, Advice and Adherence)
This is a free drop-in class held every Wednesday.* Registration is not required.
