Pharmacists and nurses will coordinate care among your home, your physician’s office and the hospital to assist in making transitions easier throughout the health care system and to ensure that all home infusion needs are met at home.

• Delivery — products are delivered to patient’s home by licensed, courteous drivers
• Drug monitoring — specific to infusion therapy

• Intravenous antibiotic therapy
• Laboratory monitoring — specific to infusion therapy
• Medical equipment — specially designed infusion and feeding pumps
• Medical supplies — to administer infusion or enteral therapies
• Nutritional support — enteral nutrition and total parenteral nutrition
• Pain and hospice/palliative care management
• Skilled nursing — includes management and care of all types of vascular access devices and subcutaneous devices, patient and family education, and certified diabetic education and monitoring
• Specialized therapy for cardiac patients

Meeting Patient Needs at Home
Our team provides convenient, readily available care to patients throughout San Diego County. We accept referrals from patients, families, physicians and other health care professionals and then work with them to develop a personalized plan of care. Our simple, efficient referral process takes just one phone call.

Our qualified personnel are available to assist patients and their families in understanding funding and payment options. Home infusion nursing services are paid for by most managed care and private insurance plans. Home infusion pharmacy services coordinate with Sharp Home Health nursing services and a variety of community nursing agencies that work with Medicare, Medi-Cal and most managed care and private insurance plans.

Sharp Home Infusion Services are available to individuals who meet the following:

• Patient is under the care of a primary or specialty physician
• Patient-prescribed infusion therapy is judged medically/clinically necessary by the pharmacist
• Patients or caregivers require instruction and/or assistance with prescribed therapy and agree to participate in the plan of care
• Patient’s physical, medical and emotional needs can be safely met at home

Our Team
Patients receive infusion therapy from our specialized team of nurses, pharmacists, dietitians, technicians, office staff and couriers. Our pharmacists maintain standards defined by the American Society for Healthcare Systems Pharmacists and the National Home Infusion Association. Our experienced home infusion nurses are trained to perform high-tech infusion procedures and are knowledgeable and practiced in a full range of infusion therapies and venous access management. Nurses adhere to standards set forth by the Joint Commission as well as the Centers for Disease Control and Prevention (CDC) and the Infusion Nurses Society.
