The following resources are available to help hospice patients and their families.

  • Advance Health Care Directive
    An Advance Health Care Directive form can help you give instructions about your medical treatment, name someone else to make health care decisions for you, express your wishes about organ donation and designate a primary care physician.
  • Insurance Coverage for Hospice Service
    Medicare, Medi-Cal and most private health insurance plans cover hospice services, including prescription medications. Sharp HospiceCare staff will fully review and explain each patient's hospice care coverage. 
  • Hospice care can began immediately for those who qualify medically and have a written referral from their physician. For more information, please call 619-667-1900.
  • Physician Orders for Life-Sustaining Treatment
    The Physician Orders for Life-Sustaining Treatment form gives hospice patients more control over their end-of-life care. Read frequently asked questions about this form to learn more.
