Pre-Anesthesia Evaluation Services

The Pre-Anesthesia Evaluation Services (PAES) department serves patients having surgery at: 

The purpose of PAES is to facilitate the process of an admission and prevent delays on the day of surgery.

A registered nurse from PAES will call and speak with patients prior to their surgery to collect and verify pertinent health information, such as height, weight, medications and past medical history. Based on the information obtained, an appointment may be scheduled for patients to come to the PAES clinic prior to the day of their surgery for testing and, in some instances, to see one of our anesthesiologists.

To use the PAES form, fill it out on-screen, print it and bring it with you to PAES if you have an appointment. If not coming to PAES, be sure to bring the form with you the day of your surgery.
