Disease Management Program for Diabetes
If you are a Sharp Rees-Stealy patient between the ages of 18 and 75 with diabetes, you may be eligible to participate in our diabetes disease management program. Through this specially designed program, the disease management teams work in collaboration with you and your doctor to help you understand the day-to-day management of diabetes, support your diabetes treatment plan and improve your quality of life. Patients enrolled in the program receive diabetes educational materials, self-management tools and, when appropriate, telephonic health coaching to help them feel better and to empower them to take a more active role in their health management.
