Disease Management Program for Chronic Obstructive Pulmonary Disease

If you are a Sharp Rees-Stealy patient with chronic obstructive pulmonary disease (COPD), you may be eligible to participate in our COPD disease management program. Through this specially designed program, the disease management teams work in collaboration with you and your doctor to help you understand the day-to-day management of COPD, support your COPD treatment plan and improve your quality of life. Patients enrolled in the program receive COPD educational materials, self-management tools and, when appropriate, telephonic health coaching to help them feel better and to empower them to take a more active role in their health management.
