Disease Management Program Earns National Accreditation

National Committee for Quality Assurance Seal 

Sharp Rees-Stealy Medical Group’s Disease Management program has achieved full national accreditation from the National Committee for Quality Assurance (NCQA). Sharp Rees-Stealy received the Patient and Practitioner Oriented Accreditation for its congestive heart failure, chronic obstructive pulmonary disease (COPD), and diabetes disease management programs.

Through these specially designed disease management programs, Sharp Rees-Stealy disease management teams work in collaboration with patients’ doctors to help patients understand the day-to-day management of their chronic conditions, support treatment plans and improve quality of life. Sharp Rees-Stealy is the only medical group in San Diego County to receive this stamp of excellence. The programs are evaluated on 32 standards and according to the NCQA, accredited status is granted only to those disease management organizations that have excellent programs for the delivery and improvement of disease management programs and meet or exceed NCQA’s rigorous standards.

The NCQA says this accreditation conveys to both health plans and patients that the Disease Management program at Sharp Rees-Stealy is dedicated to giving patients and practitioners the support, education and other help necessary to facilitate good outcomes and good care. "NCQA's Disease Management Accreditation program is thorough and rigorous. It's designed to highlight only those programs that truly improve chronic care," said NCQA President Margaret E. O'Kane.
