Disease Management Program for Asthma

If you are a Sharp Rees-Stealy patient with asthma and between the ages of 5 and 64, you may be eligible to participate in our asthma disease management program. Through this specially designed program, the disease management teams work in collaboration with you and your doctor to help you understand the day-to-day management of asthma, support your asthma treatment plan and improve your quality of life. Patients enrolled in the program receive educational materials on asthma, self-management tools and, when appropriate, telephonic health coaching to help them feel better and to empower them to take a more active role in their health management.
