Sharp Rees-Stealy Quality Measures for Coordinating Patient Care

What was measured?
Patients reported if someone from the doctor’s office followed-up to give them their test results and if their doctor was up-to-date about any care the patient got from other doctors.

These results are based on a survey of a sample of medical group patients.

Why is it important?
When you are waiting for test results getting an answer quickly is a relief. Good medical groups will contact you quickly, whether your results are normal or not.

Your regular doctor should know about all of your health care. This includes communicating with a specialist if your need a consultation.

Sharp Rees-Stealy Score: 79.74 percent
State Average: 76.23 percent
