Doctor's Visit Checklist for Seniors

Have you ever forgotten to ask your doctor an important question during your visit?

You can improve communication with your physician about your health issues by preparing some information before your visit. It is important to provide as much information as you can so that you can get the best advice from your doctor.

If this is your first visit

Ask to have copies of your medical records available for your new physician before your visit. This should include information about past illnesses, surgeries, conditions, medications and treatments.

Write down names and doses of prescription medication, over-the-counter medication, vitamins, supplements and herbal remedies.

Be sure to tell your physician about past food and drug allergies or side effects of medication.

Bring a list of questions. Don't be shy about preparing a list of questions for your doctor visit; doctors will welcome your interest. Do some research about your condition before your appointment.

Here are some sample questions:

  • What is my diagnosis?
  • What are my treatment options?
  • Should I use ice or heat?
  • Should I take my medication with food or without?
  • Are there any foods or juices that I should avoid with my medication?
  • What are the side effects of my medication?
  • What kinds of costs are involved in my treatment?
  • Does my insurance cover my treatment?
  • Does my insurance cover this medication? Is there a generic?
  • When should I receive my test results? Should I call to get them?
  • If I have other questions, how can I contact you?
  • When should I make my next appointment?

Bring your medical information

Your doctor may not have all of your current medical information, especially if this is a new condition. You should provide the following information:

  • A list of your symptoms (rank them by importance)
  • I have had these symptoms since (insert date)
  • My pain is (sharp, dull or achy)
  • Any changes in body temperature, heart rate or breathing 
  • Recent test results
  • Drug or food allergies, reactions
  • Names and doses of prescription medication, over-the-counter medication and herbal remedies and supplements
  • Alcohol consumption, smoking habits
  • Issues in the past that have kept you from taking medication or being healthy (forgetting to take medication, unable to afford the medication or treatment, transportation issues, access to nutritional food)
