Project CARE for Seniors

Project CARE (Community Action to Reach the Elderly) for Seniors is a free service for elderly or disabled adults who live alone. Unique to San Diego County, this service is a collaboration of several different community agencies and organizations. Project CARE East County is offered through Sharp Grossmont Hospital and the Senior Resource Center.

Remaining independent is important to older adults and those with special needs. But many feel as if their independence is threatened because they are alone and isolated. In reality, there are neighbors, and those that work in the community (law enforcement, postal workers, utility providers, delivery personnel), who easily can lend an eye, ear or hand to help.

How We Help*

  • Daily phone calls: Calls are placed seven days a week at a time chosen by the participant. During the week, responses are monitored by the Sharp Grossmont Hospital Senior Resource Center staff and volunteers; on weekends, responses are monitored by hospital security personnel. If a call goes unanswered, the person designated by the Project Care client will be called to check on the client. This service is free and available to anyone in East San Diego County.

  • Vial of Life: The Vial of Life is a small container that can be placed on your refrigerator door that holds health-related information such as the your name, address and medication history. If paramedics arrive in an emergency, this information assists them in providing a fast and accurate medical response.

  • Safe Return: This program, run by the Alzheimer’s Association, aids those with dementia and other cognitive impairments. Through a system of identification products (including wallet cards, ID bracelets, clothing labels, lapel pins, bag tags), and a national photo/information database, individuals who wander and get lost can be found and assisted.
