Every Sharp Community Medical Group (SCMG) office maintains a secure records system that conforms to the laws and regulations of the state of California governing retention of patient health records and the release of medical information.
How you can access your medical records.
Your medical records will be kept confidential and can only be released according to the following approved policy and procedures:
SCMG office staff and providers may release your medical record information only if a signed consent has been obtained from you or the person legally responsible for making your medical decisions. A parent or legal guardian can authorize the release of health records for minors in their care.
Any person making copies of member record information will note the release in the departmental, medical or computer record and sign and date the entry.
