Mammogram Reminder

Since the advent of annual mammogram screenings, the number of women who die from breast cancer has been reduced by 50 percent. If you are more than 40 years old, yearly mammograms — along with monthly breast self-examinations and a clinical breast exam — increase the chance that you will identify breast changes early on. Contact your primary-care physician immediately if you notice a lump or other breast abnormality.

Let Sharp send you an e-mail reminder to schedule your annual mammogram on the date you request. You may discontinue this service or change your registration information at any time.

Mammogram Reminder Instructions
Please complete the following information to personalize your mammogram reminder. The American Cancer Society recommends that all women more than 40 have a mammogram and clinical breast exam once per year, more often if you are at high risk for breast cancer. If you have had a mammogram in the past year, you may want to be reminded to make your next appointment for the same date. Please choose the best date for you.
