Continuing Care (Aftercare) Program

The Continuing Care component of treatment is crucial to recovery and beings immediately after the rehabilitation program. The goal of the program is to help you problem-solve issues and make the necessary transitions at home, work and in the community. It is important to view this as a crucial treatment commitment.

Continuing Care consists of weekly group sessions for individuals and monthly group sessions for family. Therapy sessions address ongoing issues that emerge during recovery process including relapse prevention. Patients are encouraged to participate in hospital sponsored and community 12-step meetings as well as other recovery support groups. Those who use it wisely have better outcomes and are knowledgeable in spotting their relapse warning signs.

Sharp Vista Pacifica’s Continuing Care program is available at no cost to patients or family members and participants can attend on an unlimited basis. An alumni association is also available to maintain group cohesion and relationships with other members.
