This virtual workshop will discuss the impact your spirituality and religious beliefs might have on your or your family's future heath care decisions. A specially trained Sharp HealthCare professional will review some of the basic, but important aspects of the advance care planning process, which assesses your personal beliefs, values and health care goals so that you can make important decisions with comfort and ease.
You will receive information on five helpful precepts (ideas) that might give you clearer insight as you develop your advance care plan. The instructor will also provide a brief review of an advance health care directive called Five Wishes that can help align your spiritual beliefs with your health care choices.

- Helpful guidance regarding the careful selection of a substitute health care decision-maker
- Tools that you can use to have meaningful conversations with your loved ones
- A brief review of an advance health care directive and important reminders about completion
- Information on how to have your spiritual beliefs reflected in your advance health care directive
