A specially-trained Sharp HealthCare professional and veteran will provide an overview of the ACP process, which assesses your personal goals, values and beliefs.
The workshop will also provide you with:
- Communication tips to help you have a conversation about caregiving for a veteran
- Options and information regarding Veterans Affairs' resources that may be available
- Tools to help you retrieve a lost DD214 (discharge paperwork)
- Information on creating a document to detail your wishes in advance
