If you are a veteran or caring for one, there are special considerations you should know about when discussing advance care planning (ACP). Who would speak on your behalf if you were unable to speak for yourself? What benefits or resources are available to help provide care?
Don't wait until it's too late to get valuable information you need to make important life decisions. Attend this free virtual seminar and learn how to create a plan of action. A specially-trained Sharp HealthCare professional and veteran will provide an overview of the ACP process, which assesses your personal goals, values and beliefs.
The workshop will also provide you with:
- Communication tips to help you have a conversation about caregiving for a veteran
- Options and information regarding Veterans Affairs' resources that may be available
- Tools to help you retrieve a lost DD214 (discharge paperwork)
- Information on creating a document to detail your wishes in advance

Presenter Information
This educational workshop is presented by Teressa Vaughn, MPT, MHA, an advance care planning consultant with the Sharp Advance Care Planning Department. She is also a retired veteran.
