A cancer diagnosis brings you to a crossroads in life. Do you have a road map for handling changes in your health?
Even if you aren't sick now, it may be difficult to think about planning for your future health care, but preparing a comprehensive action plan will help ensure you receive the care you want if you become seriously ill — even if loved ones need to make decisions for you.
Learn how to create a plan of action that can grow and change along with your condition at this free online workshop. A specially trained Sharp HealthCare professional will provide an overview of the advance care planning (ACP) process, which assesses your personal beliefs, values and health care goals so that you can make important decisions with comfort and ease.
The workshop will also provide you with:
- Basic tools to help define your own health care choices
- Communication tips you can use to help start the conversation with your loved ones
- The opportunity to work on your health care directive with guidance from an ACP facilitator
Registration and a valid email address are required. An email will be sent to you with the WebEx login and/or call-in information by the morning of the workshop.

What to Bring
If you have a completed health care directive, feel free to have it with you as you may want to update it during the workshop.
