Who can make health care choices for you when you can't? Don’t wait until it’s too late to make important decisions about your medical care. Attend this free seminar to learn about the importance of completing an advance health care directive — a document that lets your physician, family and friends know your health care preferences when you are unable to make such decisions on your own.
You will hear about the many advance care planning resources in the community and will be able to receive assistance with completing your own directive and a notary will be on site to certify it for free. Lunch will be provided.

Parking is available in the structures at the front and back of the main hospital, as well as in the large lot behind the main hospital. Paid valet parking is available to patients and visitors Monday through Friday, 10 am to 4 pm.
If you already have an advance directive, you can bring it to the seminar and have a notary certify it for free.
