This webinar will provide family caregivers with an overview of the advance care planning process.

Providing care for a friend or loved one can be a rewarding yet daunting task. Is there someone who knows what resources are available and can honor your wishes to see them through? What if you have no one to speak on your behalf? What if you find yourself having to care for an estranged loved one? If you had an unfortunate life event such as a stroke or some form of dementia, who would care for you? Where do you live? What resources are available?
Learn how to create a plan of action that can address caregiving challenges at this free virtual workshop. A specially-trained Sharp HealthCare professional will provide an overview of the advance care planning (ACP) process, which assesses your personal beliefs, values and health care goals, so that you can make important decisions with comfort and ease.
The workshop will also provide you with:
- Basic tools to help you think through the process of day-to-day caregiving when/if needed
- Communication tips to help you have a conversation about caregiving with your loved ones
- Various caregiving options and community resources that may be available
- Information on creating a document to detail your wishes in advance

Presenter Information
This educational workshop is presented by Teressa Vaughn, MPT, MHA, an advance care planning consultant with the Sharp Advance Care Planning Department. She is also a full-time caregiver for her mother with advance Alzheimer's dementia.
