San Diego Health Connect Medical Information Exchange
Sharp HealthCare is proud to participate in San Diego Health Connect, our community's medical information exchange. This electronic exchange allows participating hospitals and health care providers throughout San Diego County to safely and securely share patient health information — resulting in a more complete, up-to-date medical record that helps patients receive the best care possible.
The electronic exchange is designed to further enhance your health care experience by reducing the likelihood of undergoing redundant tests and procedures, and lessening your burden of keeping track of and transferring your medical documents to each of your doctors.
Sharp HealthCare began sharing patient health information electronically through San Diego Health Connect in mid-July 2015. Sharp will only send patient health information through San Diego Health Connect if and when you seek treatment from a doctor outside of Sharp who participates in the community health information exchange. Your information will not be shared unless there is a request from a doctor outside of Sharp who is caring for you.

Opt out of participating in San Diego Health Connect.
If you do not want to have your Sharp medical records made available through San Diego Health Connect, please complete the opt-out form at the bottom of this page.
Any information that is shared with San Diego Health Connect before you submit this opt-out request may remain securely with health care providers who accessed information before this opt-out went into effect. Your opt-out request will be effective approximately five business days after submitting the information below.
By completing this form, you request that Sharp not share your medical record electronically through San Diego Health Connect health information exchange with any other regions or outside health care organizations. If you choose not to participate, none of your Sharp HealthCare medical information will be shared electronically through San Diego Health Connect; however, in the event you see a caregiver for treatment outside of Sharp, that provider may request and receive your medical information from Sharp through other methods, such as fax or mail.
If you decide to opt back in to San Diego Health Connect, you will need to submit the to the medical records/health information management department at any Sharp hospital opt-in form (PDF) or Sharp Rees-Stealy medical office. We also have a Spanish version of the opt-in form (PDF) available for you.
A separate request must be completed for each family member wishing to opt out.
How to find this number
Your Sharp HealthCare number can be found in the notification letter that you may have received in the mail in March or April 2015, on your Sharp billing statement, on a Sharp hospital discharge summary and on your Sharp Rees-Stealy patient identification card.
If you are not able to locate your number, please call 858-499-2415, Monday to Friday, 8 am to 4:30 pm to complete the opt-out process.
