Our Advance Care Planning (ACP) Program is a free service to help you plan for the future. Trained ACP consultants will help you prepare a comprehensive plan of action detailing your personal preferences regarding health care decisions. We cover all issues that are important to you, including:
- Appointing a health care agent to carry out your wishes in the event that you are unable to do so
- Developing a communication plan and dialogue to discuss your preferences with friends and family
- Outlining your values, personal concerns and cultural or spiritual beliefs
Ensure your advance care plan will be carried out.
Once your advance care plan is in writing, make sure that your physician, friends and family understand what your preferences are and what is included in your advance directive. We'll be by your side every step of the way, offering the support you need to make your wishes understood.
