To ensure that you and your family get the information and care you need, home health liaison nurses at the hospital are available to work with you in evaluating your situation and facilitating a smooth transition to home.
Medicare, Medi-Cal and most managed care and private insurance plans pay for home care services. Our staff members are available to assist you and your family in exploring financial resources as needed.
Eligibility for home care.
Sharp Home Health services can be accessed from a variety of settings, whether one is at home or is being discharged from a hospital or a skilled nursing facility.
Home health services are available to individuals of all ages with new or chronic conditions who meet the following guidelines:
- You are homebound
- You are under the care of a physician
- You require services on an intermittent basis
- Your physician orders and specifies the type of services to be provided
- Your requested services are deemed reasonable and necessary by your insurance company
