Sharp Community Resource Center

We offer personalized consultations to connect you with the right resources to help care for you and your loved ones. These resources may include assisted living facilities, private caregiving companies, transportation options and meal services.

Additionally, we can help connect you with educational programs and other community services that can enhance your quality of life. Our goal is to ensure that you have access to the most relevant and up-to-date information to make informed decisions and find the support you need.

Are you providing care for a loved one? If you are new to or have long embraced the important role of caregiving, the Community Resource Center has classes and services to support you.

• The Caregiver Basics Workshop is a free three-part series for family caregivers run by a team from Sharp HealthCare, including physical therapists, occupational therapists and social workers. Classes cover proper transfer techniques, managing medications, caregiver self-care and community resources.

• The Caregiver Support Group is designed for you to share resources, experiences and emotions in a safe environment. Led by social workers from Sharp Grossmont Hospital, this free support group meets the first Wednesday of each month and is open to both new and long-term family caregivers.
  

The Community Resource Center hosts regular fitness classes led by a trained instructor to promote health and well-being. These group classes are a fun and informal way to exercise, meet new people, and learn easy ways to maintain strength and balance.

• The Sharp Checks In program offers wellness checks and medication reminders by phone for older adults living at home independently. This free service allows older adults to maintain their independence, while providing peace of mind for concerned family and friends.

• The Vial of Life provides a reliable method for documenting essential medical details that first responders know to look for in the event they are called to your home for a medical emergency.
