Proxy Accounts: Access to a Loved One's Health Information
It's easy for you to manage your family's health care as well as your own using FollowMyHealth®.
Manage health care for minor children and dependent adults.
For Sharp Rees-Stealy patients only: At your dependent's next doctor's visit, fill out and submit an authorization form allowing you online access to his or her health information. You may choose to download and submit the authorization form (PDF) now instead of waiting for a doctor's appointment.
Parents of newborns: If you deliver a baby at a Sharp hospital, you will automatically receive an email invitation to connect your FollowMyHealth account to your baby's health information. If you do not already have an account, you will need to set one up first.
Limited access for children ages 12 to 17: The Health Insurance Portability and Accountability Act of 1996 (HIPAA) gives minors 12 years of age or older the legal right to confidential medical care. To comply with the law, FollowMyHealth limits what health information parents and guardians can view for children over 12.
Grant access for another adult to your account.
If you would like to give someone proxy access to your account — such as your spouse or loved one — follow these steps from your desktop computer (you cannot request proxy from a mobile device):
- Log in to your own FollowMyHealth account
- From the main screen, go to "My Account" and choose "My Connections"
- Select "With Family Members, Dependents, etc."
- Select "Add Authorized Individual"
- Follow the instructions provided
You may grant read-only access or full access, which allows your loved one to interact on your behalf including making appointments and messaging your physicians.
