Beginning Sept. 17, Sharp Rees-Stealy (SRS) team members can get vaccinated at any SRS Occupational Medicine clinic locations from 8 am to noon and again from 1 to 4 pm. Please note these locations close for lunch from noon to 1 pm. Additional SRS employee vaccination will begin Oct. 1.

Flu vaccination documentation

Sharp team members who receive their vaccine through the Employee Occupational Health Department (EOHD) or SRS Occupational Medicine will automatically have their EOHD record updated.

When vaccinated at retail pharmacies or through your personal care provider, you must provide EOHD with a copy of your documentation in order to update your compliance and to receive a 2025 Flu sticker. Documentation can be provided in person or by email to EOHD. Please note that your personal health record (Sharp account) does not link to your employee health record.

  • Flu sticker At the time of vaccination, team members are given a flu sticker that they may voluntarily place on their badge to be easily recognized as vaccinated. Team members who submit flu vaccine documentation by email to EOHD must pick up their flu sticker in EOHD.

  • Declination — Team members who decline flu vaccination must complete an online declination form.

  • Compliance — All team members are required to either be vaccinated, provide documentation of vaccination received elsewhere, or decline the flu vaccine by Oct. 31.

COVID-19 vaccination and policy requirement

We are currently awaiting guidance from professional societies and federal, state and local public health authorities. We will send additional communication once this has been determined. The policy requirement to get vaccinated, attest to outside vaccination, or decline this season’s vaccine is still in place because this information is reported with our facility licensure and regulatory surveys.

  • Vaccination EOHD is currently awaiting COVID-19 vaccination guidance for the 2025-26 season from professional societies and federal, state and local public health authorities. Currently, EOHD is not providing the COVID-19 vaccine, but if that changes, additional communication will be sent.

  • Documentation — If you were vaccinated at a retail pharmacy or through a Sharp provider, you must provide EOHD with a copy of your record to update your compliance. Please note that your personal health record (Sharp account) does not link to your employee health record. Documentation can be provided in person or by email to EOHD.

  • Declination — Team members who decline this season’s COVID-19 vaccination are required to complete an online declination form. If team members were previously approved for an exemption prior to 2024, nothing needs to be done.

  • Compliance — All team members are required to either be vaccinated, provide documentation of vaccination received elsewhere, or decline the COVID-19 vaccine by Oct. 31. Please allow 48 to 72 hours for compliance to reflect in Workday and Tableau Reports.

  • Masking — There is no masking requirement related to the COVID-19 vaccine. However, during periods of increased respiratory disease transmission or other related metrics, all team members will be required to follow other mitigation measures, like masking or distancing, as directed by Sharp HealthCare, local, state or federal agencies.
