Sharp Extended Care offers an alternative to traditional skilled nursing care by coordinating short-term rehabilitation services in a warm, homelike environment.
Here, full-time Sharp-affiliated doctors and a nurse case manager help patients transition from the hospital to home and coordinate ongoing care. This innovative approach is designed to help patients go home faster and prevent unnecessary returns to the hospital.

At Sharp’s skilled nursing facilities, our expert team of registered nurses, licensed vocational nurses, certified nursing assistants, social workers and registered dietitians works together to develop a comprehensive care plan for each patient. And we’re here to provide support to any needs your family may have — from coordinating activity plans to serving as an advocate for your loved one.
Our on-site medical director is available 24/7 to assist residents when needed. And if hospitalization is required, we can provide immediate access through Sharp's comprehensive network of acute-care hospitals.
Sharp’s skilled nursing facilities all maintain a residents' council that meets regularly for residents and their families to discuss issues or concerns within the facility. All residents are encouraged to attend, and residents unable to speak for themselves are often represented by family members who attend with them.
A wide variety of rehabilitation therapies are available to patients with impaired functional abilities due to stroke, brain injury, spinal cord injury or other neurological conditions. Patients with disabling conditions — including amputation, severe arthritis and those caused by orthopedic or cardiac conditions — can also access our highly qualified physical, speech and occupational therapists.
Many who have undergone surgery may no longer require acute hospital care but are not yet ready to return home and require more services than can be provided through home health care. A skilled nursing facility can provide these patients with the specialized recuperative care they need and improve their transition from surgery to home.
Our subacute nursing units offer comprehensive care for patients recuperating from surgery, strokes, broken limbs, persistent sores and other conditions. Patients eventually go home, though some do transition to other, more appropriate levels of care, as determined by their care team.
